How does the intake process work?

We are so pleased that you have contacted us to learn more about the intake process for our clinic. If you are interested in only therapy at this time, you may go to the “Contact” tab and let us know in the message area that you are interested in scheduling a therapy appointment. Please be aware that the therapists have little to no availability at this time. If you are interested in medication management, please take a few moments to read through the following information and submit the requested information within the form below. If you are interested in therapy and medication management, there will be a place within the form to give that information.

We want to offer everyone a safe and peaceful treatment experience that is tailored to each person’s specific needs. Thank you for taking a little extra time to thoughtfully read through this information and to answer the questions in the form below. This attention to detail helps us to create a more thorough profile for you, which allows providers to access the most beneficial resources and treatment options.   • Our nurse practitioner sees patients beginning at age 6 through adults.

  • We take most major insurances, but please check with your insurance provider to verify that we are in network. Please be aware that we DO NOT take Medicare or any of its agencies and CANNOT accept other forms of insurance if you also have Medicare. You may always choose self-pay as an option.

  • If you have previously been prescribed psychiatric medications, have ever received inpatient psychiatric care, or take regular medications for any health conditions, we will need those records, and are happy to facilitate requesting those on your behalf.

  • We do not offer ADHD testing. If you are interested in medications for ADHD and are not currently being treated, we do require testing results or records showing prescribed ADHD medications that is less than five years old. We do not accept online testing. Testing must be provided by an in person, licensed psychologist or psychometrist. You may check online, with your insurance, or we can send a list of recommended testing providers.

  • If you will need to obtain records, please indicate that in the form below so that a release form can be sent to you allowing us to request your medical records on your behalf.

  • Our nurse practitioner is not offering a telehealth option at this time, although some of the therapists do offer telehealth visits.

  • When you have submitted the intake form, and records have been received if needed, you will be contacted to schedule your appointment. I will keep you updated with the progress as we move forward in the process.

  • If you have questions or concerns about the costs of evaluations or visits, or any other points that may need more clarification for you, please leave those in the comment box at the end of the form, or contact the office.
